This Act enters into force on the date of the last signature, unless a different effective date is stated in Art. 1.
The parties agree to modify the Original Contract as follows (tick applicable): ☐ Extend the lease term until [new date]. ☐ Adjust the monthly rent to [amount] + VAT (if applicable), effective [date]. ☐ Change the payment due date to the [day] of each month. ☐ Modify the utility billing method (e.g., from flat fee to actual consumption). ☐ Add/remove a co-tenant or guarantor: [name]. ☐ Change the property’s permitted use (e.g., from residential to office).
This Act is made in two original copies, one for each party. It is signed in private writing, unless the law requires authentic form (e.g., for a term exceeding 3 years – Art. 1799 Civil Code).

All other terms and conditions of the Original Contract remain in full force and effect. In case of conflict between this Act and the Original Contract, this Act shall prevail for the matters expressly listed in Art. 1.
